The position will be based at Bennerley Fields with coverage across Ilkeston and Ripley Hospitals within the Community Therapy

Responsibilities

• Provide rehabilitation and complex discharge planning to patients.
• Support 7-day service with some weekend and bank holiday coverage (approx. 1 in 6 weekends).
• Work across Bennerley Fields, Ilkeston Hospital and Ripley Hospital as part of the Community Therapy Service.
• Integrate into MDT and contribute to a culture of continuous improvement and patient-centered care.
