Job summary

LinsCare in Wales is seeking an experienced Occupational Therapist to join the Community Occupational Therapy Team in Ceredigion. You will provide high-quality, person-centred therapy to adults and older people, helping maximise independence, safety and quality of life.

This role involves managing a varied caseload, conducting assessments, advising on equipment and housing adaptations, and working with multi-agency teams.

Qualifications

• HCPC registration as an Occupational Therapist.
• Experience in community occupational therapy with adults/older people.
• Knowledge of specialist equipment, housing adaptations and manual handling.
• Ability to manage a complex caseload and work autonomously.
• Excellent communication and multi-agency working skills.
• Commitment to person-centred practice and safeguarding.
• Willingness to undertake community visits and flexible hours.

Responsibilities

• Manage a varied and complex caseload independently.
• Undertake functional, manual handling and risk assessments.
• Assess and recommend equipment and housing adaptations.
• Develop person-centred intervention and care plans.
• Provide advice and support to individuals, families and carers.
• Collaborate with health, social care and partner agencies.
• Supervise and delegate to OT Assistants, students and volunteers.
• Maintain accurate records and prepare professional reports.
• Participate in duty rotas, supervision, training and ongoing professional development.
