Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking a Registered Nursing Associate to join the Integrated Community Team in Swadlincote. You will provide safe, person-centred nursing care in community settings, working with nurses and the multidisciplinary team, and contribute to care planning, delivery, and evaluation.

The role offers variety and potential progression via the Nursing Apprenticeship, with service cover seven days a week.

Qualifications

Registered Nursing Associate on the NMC register.
Level 5 Foundation degree specific to Nursing Associate practice.
Placement experience working within the 4 fields of nursing.

Responsibilities

Provide and monitor nursing care for a designated group of patients.
Work within the Nursing Associate scope of practice and the NMC Code.
Recognise changes in a patient's condition and elevate concerns accordingly.
Follow agreed care plans, clinical procedures and treatment plans.
Organise and prioritise a delegated workload, responding to changing or unpredictable needs.
Communicate effectively with patients, families, carers and colleagues.
Work collaboratively with Registered Nurses and the multidisciplinary team.
Maintain accurate and timely clinical records using electronic systems.
Participate in reflective practice, clinical supervision and NMC revalidation.
Support Nursing Associate students, other learners and healthcare support workers.
