Job summary
Royal Devon University Healthcare NHS Foundation Trust is seeking a Registered Nurse for a community nursing role based in Exmouth and surrounding areas. You will assess, plan, implement and review patient care in the home or community settings, promoting independence and quality of life.

Qualifications
- Registered Nurse with appropriate nursing qualification and registration.
- Experience in community nursing is desirable but training provided.
- Ability to assess, plan, implement and review nursing care in home/community settings.
Responsibilities
- Undertake nursing assessments and deliver patient-centred care.
- Support self-management and long-term condition care to prevent hospital admission.
- Deputise for Community Specialist Nurse when required.
- Work within Community Nursing Team to meet patient needs in home or community settings.
