Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a senior community nurse to lead a caseload of patients with complex health needs, delivering holistic assessments and steering care planning within patients’ homes and community settings.

The post emphasizes clinical leadership, safe discharge planning and proactive collaboration with multidisciplinary teams to promote independence and high-quality outcomes for patients living at home.

Qualifications

• BSc Nursing or equivalent.
• Post-registration qualification or university degree.
• ENB 998 or PETALS mentorship qualification.
• Management or leadership qualification or development programme.

Responsibilities

• Manage a caseload of patients with complex health needs and provide holistic assessments.
• Provide clinical leadership and support to the wider nursing team.
• Monitor quality of care planning, implementation and evaluation.
• Ensure medicines are handled and given safely in line with guidelines.
• Plan discharge and transfer with the multidisciplinary team.
• Help prevent unnecessary hospital admissions and keep patients independent at home.
• Build relationships with the multidisciplinary team, patients, families and carers.
