Job summary

NELFT North East London Foundation Trust is seeking a Community Staff Nurse to deliver high-quality nursing care in the community across North East London. The successful candidate will be a Registered Nurse (RN12-NMC), assess and plan care, and work as part of a multidisciplinary team to promote patient independence and wellbeing.

The role requires providing person-centred care, maintaining records, and contributing to service improvements.

Responsibilities

• Assess, plan, implement and evaluate individualised nursing care for patients with acute, chronic and palliative care needs in the community.
• Deliver safe, effective and evidence-based nursing care in accordance with professional standards, Trust policies and clinical guidelines.
• Recognise changes in patients' conditions, identify clinical risks and take appropriate action, seeking advice or escalating concerns when necessary.
• Maintain accurate records and ensure compliance with Trust policies.
• Work collaboratively with GPs, nursing colleagues, allied health professionals, social services, acute hospitals and voluntary organisations to support service and coordinated patient care.
• Supervise pre-registration students.
