NELFT NHS Foundation Trust is seeking a motivated Registered Nurse to join our Integrated Community Team in Waltham Forest South. You will assess, plan and deliver evidence-based nursing care to patients in the community, helping them maintain health, independence and dignity.

Role involves working with GPs, allied professionals and social care to coordinate care; requires NMC registration, driving licence, and willingness to work weekends/Bank Holidays. Outer London supplement applies.

Qualifications

• NMC Registration to practise in the UK.
• Experience in community/primary care nursing is preferred.
• Ability to work independently and as part of a multidisciplinary team.

Responsabilités

• Assess, plan, implement and evaluate individualised nursing care in the community.
• Deliver safe, evidence-based nursing care according to standards and guidelines.
• Recognise changing patient conditions, escalate concerns and coordinate with teams.
