Job summary

Swansea Bay University Health Board is seeking a registered nurse to join the Primary Care and Therapies Group. The post focuses on professional, holistic nursing care for patients, carers and families in community settings, with a flexible 7-day service.

You will work as part of a multi-disciplinary team, often as a lone worker, visiting patients at home, transporting equipment between sites, and escalating concerns when needed. CPD and training are supported.

Qualifications

• Level 1 Registered Nurse (Adult)
• Current NMC Registration
• Evidence of personal continuing development, statutory and mandatory training
• Awareness of Health Board policies, procedures and guidelines
• Ability to relate theory to practice
• Understanding of primary health care working

Responsibilities

• Assess individual patient’s care needs and escalate concerns to the appropriate team member.
• Deliver professional, holistic nursing care in community/home settings.
• Work as a lone worker, delegating tasks, carrying out assessments and evaluations, and escalating concerns as needed.
• Travel independently between sites, transporting equipment and medical stock as required.
