Job summary

NHSGGC is seeking a Community Nurse within Glasgow District Nursing Services, delivering clinical interventions to patients in their homes and residential settings. The role supports the District Nurse to provide high-quality, person-centred care and to help patients maintain independence at their preferred place of care.

The post is permanent, part-time (22.5 hours per week) with a base at Govan Health Centre.

Qualifications

• Previous NHS experience is desirable.
• Knowledge of caring for patients with long-term conditions.
• Understanding of wound management and end-of-life care.
• Ability to work independently and as part of a multidisciplinary team.
• Full UK driving licence required.

Responsibilities

• Provide clinical interventions and healthcare to patients in their homes and residential care settings.
• Support the District Nurse in the delivery and management of patient care.
• Assist with the management of long-term conditions.
• Deliver wound care and end-of-life care.
• Promote patient independence and wellbeing.
• Help prevent unnecessary hospital admissions.
• Support safe and timely discharge from hospital services.
