Gloucestershire Health and Care NHS Foundation Trust is seeking a qualified Community Nurse for the Tewkesbury and Newent Team. The role is permanent and can be full-time or part-time.

Shifts run across early and late slots (8am–4pm and 12pm–8pm) over seven days, with a requirement for a full driving licence and vehicle to support community visits. • Registered Nurse with NMC registration must be held.
• Willingness to undertake additional training as required.
• Full driving licence and use of vehicle required for role.

Responsabilidades

• Assess patient and carers needs through strength based assessment.
• Develop individual care plans in partnership with patients and carers.
• Regularly evaluate care plans and report clinical changes to the team.
• Administer and monitor medications per policy.
• Maintain accurate patient documentation on the electronic system.
• Work across a shift system including early and late shifts, weekends and bank holidays.
