Job summary

Gloucestershire Health and Care NHS Foundation Trust in Gloucestershire invites a Community Nurse to join the Tewkesbury and Newent Team on a fixed-term 12-month contract. You will work in patients' homes, delivering patient-centred care and collaborating with the Integrated Community Team.

You must be a Registered Nurse with the NMC, hold a full driving licence, and be willing to undertake additional training. Shifts include 8am-4pm and 12pm-8pm across 7 days including weekends.

Qualifications

Responsibilities

• Assess patient and carer needs through strength-based assessment.
• Develop individual care plans with patients and carers.
• Maintain accurate patient documentation and update records.
• Provide nursing care interventions and support to colleagues.
• Work across a shift system including weekends and bank holidays.
• Collaborate with Integrated Community Teams to promote independence.
• Administer and monitor medications according to policy.
• Prioritise workload and manage a delegated caseload.
