Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a Community Nurse for the Tewkesbury and Newent team. The role involves home assessments, care planning, medication administration and managing a small caseload, with shifts across seven days and weekend work.

A 12-month fixed-term contract is offered, with preceptorship for newly qualified nurses. Current NMC registration and a full driving licence are essential.

Qualifications

• Willingness to undertake additional training as required.
• Current NMC registration.
• Previous community nursing experience.
• Experience of working without direct supervision.
• Experience of working with student learners in practice.

Responsibilities

• Assess patient and carer needs through strengths-based assessment.
• Develop individual care plans with patients and carers, including information, advice, signposting and education.
• Regularly evaluate care plans and report clinical changes or concerns to the appropriate team member.
• Provide skilled nursing interventions, support and advice to colleagues, patients and carers.
• Administer and monitor medications in line with policy.
• Take responsibility for a delegated caseload and contribute to prioritising workload.
• Deliver evidence-based care that meets patient requirements and satisfaction.
• Maintain accurate patient records on the electronic clinical recording system.
• Refer patients to other disciplines and agencies better placed to meet their needs.
• Collaborate with Integrated Community Team colleagues to maximise people's potential and independence.
• Work to reduce unwarranted variation in clinical practice using evidence-based interventions.
• When suitably experienced, act as a clinical triage nurse in the centralised Community Nursing referral centres
