Job summary

HCRG Care Group is seeking a Band 5 Community Nurse to join the Integrated Neighbourhood team. The post focuses on delivering care in patients’ homes or in community settings, working with a multidisciplinary team to ensure high-quality, evidence-based nursing.

The role includes supervising junior staff and contributing to quality assurance. The successful candidate will hold NMC registration, have community nursing experience, and be adept with IT systems including SystmOne.

Qualifications

• Registered Nurse with NMC registration and community experience.
• Proficient IT skills including SystmOne, Word and Excel.
• Ability to manage caseload and deliver nursing care in community settings.
• Valid UK driving license with access to a vehicle.

Responsibilities

• Assess, plan, and deliver clinical nursing care in the community.
• Work with multi-professional teams to meet patient needs.
• Manage own caseload and supervise junior staff when required.
• Ensure patient-centred care and participate in quality assurance.
