Job summary

Cornwall Partnership NHS Foundation Trust is seeking a permanent part-time Band 5 nurse to join the Watergate Community Nursing Team. In this role you will assess, plan, deliver and evaluate home-based care, working autonomously and managing your own visit list while mentoring students on university placements.

The position involves travel across Cornwall, including areas without public transport. Newly qualified nurses and students due to qualify this year are welcome, with offers conditional

Qualifications

• Registered adult nurse with current NMC registration.
• Mentoring qualification or willingness to work towards it and attend mentorship updates.

Responsibilities

• Assess, plan, implement and evaluate care for patients, carers and relatives in their homes, without direct supervision.
• Work in partnership with patients and carers to offer support, advice and informed choice.
• Prioritise and manage own patient visit list.
• Work autonomously, providing evidence-based care in line with competency and training record and organisational policies.
• Support the Community Nurse Team leader and other locality teams when needed.
• Travel frequently across the county, including to areas not reachable by public transport.

A permanent part-time Band 5 post within the Watergate Community Nursing Team at Cornwall Partnership NHS Foundation Trust.
