Job summary

South Tees Hospitals NHS Foundation Trust is seeking a Band 5 Community Nurse to join the Hambleton & Richmondshire locality, working from Herriot Hospice at The Lambert. You will deliver holistic home-based care, including wound care, continence, palliative care, health promotion, and venepuncture, within a seven-day service.

The role requires flexibility to cover other teams as needed; induction is provided and service times may be confirmed during a period of change.

Qualifications

• Registered with NMC and fit to practice.
• Diploma or degree in adult nursing.
• Desirable teaching/mentoring qualification and post-graduate training.

Responsibilities

• The Community Staff Nurse provides clinical care and supports the District Nurse Sister and Clinical Lead.
• Works with GP practices and partners to support patients with highly complex needs to prevent hospital admission.
• Adapts to changing needs of the Trust and community care models.
• Participates in a development programme to enhance professional growth.
