Job summary

South Tees Hospitals NHS Foundation Trust is seeking a Band 5 Staff Nurse to join the Community Nursing Team in the Hambleton & Richmondshire locality, based at The Friarage Hospital, Northallerton. 25 hours per week with shift patterns 08:00–18:00 and duties including wound care, continence management, palliative care and venepuncture in patients' homes and residential settings.

Qualifications

• Registered with the Nursing and Midwifery Council (NMC).
• Diploma or Degree in Adult Nursing.
• Experience of multi-disciplinary working.
• Commitment to evidence-based practice.
• Flexible and adaptable approach in work practices.

Responsibilities

• Provide clinical care and support to the District Nurse and Clinical Lead within the community teams.
• Support patients with complex needs to prevent unnecessary hospital admissions.
• Be prepared to cover across other teams within the locality as service needs arise.
