Job summary

NELFT NHS Foundation Trust is seeking a Band 5 Community Nurse to join the CHCS across Havering. You will deliver holistic care in patients' homes, residential care settings, and nurse-led clinics, working within your competency and with a supportive team.

The role requires a registered nurse (NMC Part 1) and a willingness to collaborate across professional boundaries, liaising with GPs and other agencies to ensure high-quality, evidence-based care.

Qualifications

• Registered nurse with current NMC registration.
• Full UK driving licence and access to a car.
• Experience in community nursing or primary care is advantageous.

Responsibilities

• Provide holistic health care to patients in their own homes and in nurse-led clinics within Havering.
• Work as part of the integrated community health care team, liaising with GPs and other professionals.
