Gloucestershire Health and Care NHS Foundation Trust is seeking committed Band 5 nurses with community experience to join the Severn Health Team in Stonehouse. You will provide holistic, patient-centred care in patients' homes and clinics, and manage a varied patient caseload.

Newly qualified nurses are welcome. The role offers a rolling weekend rota (usually 1 in 4) and a supportive, family-friendly working environment.

Formación

• NMC registration (adult, physical health) required; newly qualified welcome.
• Experience in community nursing preferred but not essential.
• Full UK driving licence required to meet role needs.
• Computer literacy advantageous.

Responsabilidades

• Practice as a member of the community nursing team within ICTs.
• Liaise with GPs, ICTs and statutory/voluntary agencies.
• Provide information and education to patients and carers for independence and wellbeing.
• Develop and implement holistic care plans with patients and carers.
• Administer and monitor medications per policies.
• Maintain accurate patient documentation in the electronic records system.
• Act as Named Nurse for a defined cohort when experienced.
