Job summary

Wight Healthcare seeks a motivated Community Nurse to deliver high-quality, patient-centred care across the local community. Working autonomously within professional guidelines, you will assess, plan, implement and review care for a defined home-based caseload.

You will collaborate with multidisciplinary colleagues, coordinate referrals, and focus on preventing avoidable hospital admissions through health promotion, rehabilitation and disease prevention.

Qualifications

• Current NMC registration as a nurse.
• Full UK driving licence and access to a vehicle for work.
• Experience in community/district nursing is desirable.
• Ability to work autonomously within professional guidelines.

Working autonomously within professional guidelines and accountability frameworks, you will manage a defined community caseload, providing holistic assessments, care planning, treatment and ongoing support to patients in their own homes and community settings.

You will work collaboratively with multidisciplinary colleagues to coordinate care, manage referrals according to clinical need, and ensure excellent standards of patient safety, documentation and clinical outcomes. The role plays a key part in preventing avoidable hospital admissions, supporting timely discharges, and promoting independence through rehabilitation, reablement, health promotion and disease prevention initiatives.
