Qualifications

• Registered nurse with current license.
• First degree in Nursing or related field.
• Valid driving licence and competent driver.

• Up-to-date community experience.
• Case management experience.
• Experience working with a multi-disciplinary team.

Responsibilities

• Participate in assessment and care planning with Team Manager and specialists.
• Implement and evaluate care per care plan; report deviations.
• Communicate effectively with patients, carers and the MDT.
• Prioritise daily work based on clinical/patient need.
• Maintain accurate patient records (electronic and paper).
• Implement evidence-based care and best practice.
• Attend team meetings and daily handover.
• Initiate health promotion with individuals and carers.
• Contribute to service developments and initiatives.
• Participate in clinical audits and implement changes.
