Job summary

Kent Community Health NHS Foundation Trust in Herne Bay is seeking a Band 5 Community Nurse to support housebound patients across Dover and surrounding areas. The role focuses on hands-on nursing at home, including wound care, catheter care, injections, venepuncture and palliative care.

You will work as part of a multidisciplinary team, plan and coordinate care, and provide long-term condition support while aligning with Primary Care Networks.

Qualifications

• Customer-focused approach to give patients and families the best possible experience.
• Ability to use IT systems and databases.
• Signposting patients to other services to support behaviour change and improve health.
• Up‑to‑date clinical practice following evidence‑based procedures and clinical effectiveness frameworks.
• Understanding of the role and remit of a healthcare worker.

Responsibilities

• Deliver nursing interventions in patients' homes, including wound management, catheter care, injections and venepuncture.
• Provide palliative and end-of-life care.
• Manage symptoms and support patients living with long-term conditions.
• Educate and empower patients to maximise independence and avoid unnecessary hospital admissions.
• Assess, plan and coordinate care for patients with complex health needs.
• Work as an integral member of a multidisciplinary team.
• Plan and organise own workload with support from experienced colleagues and clinical leadership.
