Job summary

HCRG Care Group is seeking an Associate Practitioner to join the Community Nursing Team at Gravesham Community Hospital, Gravesend. You will support patients at home, undertake delegated clinical tasks and work with nurses to promote independence and better outcomes.

The role offers development opportunities within community healthcare, with access to eLearning, a supportive team, and a clear pathway to further responsibilities as you gain experience and qualifications.

Qualifications

• NVQ Level 3 in Health and Social Care or equivalent healthcare qualification.
• Experience in healthcare, ideally community nursing or related services.
• Capability to support clinical interventions and medications as trained.

Responsibilities

• Deliver delegated clinical interventions under guidance of registered nurses.
• Record clinical observations accurately.
• Support wound care and dressing changes after competencyassessment.
• Assist with medication administration and management.
• Monitor patient progress and escalate concerns when needed.
• Support patients with long-term conditions and complex needs.
• Maintain electronic patient records in a timely manner.
• Collaborate with Community Nurses, GPs and other colleagues.
• Promote health, wellbeing and independence.
• Contribute to service improvement and high standards of care.
