Main duties of the job
  1. Manage referrals and provide evidence-based care to support patients at home.
  2. Work alongside general practice, therapy, and home visiting colleagues.
  3. Assess, plan, implement, and evaluate personalized patient care, promoting self-care while maintaining dignity.
  4. Support junior staff and assist the community nursing team in managing the healthcare environment.
