Job summary

HCRG Care Group in Keynsham Health Centre is seeking a Community Nurse Band 5 to deliver high-quality, person-centred nursing care in patients' homes, under AfC terms with NHS Pension. The role is based at Keynsham Health Centre, 37.5 hours per week.

You will work with GPs and multidisciplinary teams to assess needs, plan and deliver care, and support continuous improvement. This fixed-term contract lasts 1 year, with driving and IT skills essential.

Qualifications

• NMC/1st level registered nurse.
• Ability to work independently and within a multidisciplinary team.
• Full UK driving licence with access to a car.
• Strong communication and organisational skills.
• Proficient with Microsoft Office (Outlook, Word, Excel).

Responsibilities

• Assess, plan, deliver and evaluate nursing care with patients and carers.
• Monitor patient progress and escalate changes when needed.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvements.
• Promote health, wellbeing and independence for patients and carers.
• Support learning and development of students and colleagues.
• Ensure safe environment through infection prevention and control standards.
• Maintain safe use and maintenance of clinical equipment and stock.

Job Introduction

As a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes .

Hours:37.5 hours per week. Please note that this is a fixed-term contract for 1 year.

As a Community Nurse you will require excellent communication, organisation, and clinical skills, alongside a commitment to delivering compassionate care and continuously developing professional practice:

• Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
• Monitor patient progress, recognising and escalating changes in condition appropriately.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate and timely patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
• Promote health, wellbeing, and independence for patients and carers.
• Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
• Support the learning and development of students and colleagues.
• Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
• Ensure the safe use, maintenance, and management of clinical equipment and stock.

The Ideal Candidate

You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. It is expected that youshould have strongcommunication and organisation skills and a flexible approach which will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey.

You will have a full UK driving license with access to a car for work purposes with Business insurance for the vehicle used.

You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i .e. Outlook, Word, Excel.
