Job summary

Provide CIC is seeking registered nurses to join our Integrated Care Teams and participate in the assessment, implementation and evaluation of home-based care for patients. You will contribute to developing the Primary Health Care Team and deliver high-quality clinical support in patients' homes.

• Registered nurses with valid nursing registration.

Job description

We are looking for registered nurses to work for us as part of our Integrated Care Teams and will participate in the assessment, implementation and evaluation of care to patients in their own homes and will be committed to the development of the Primary Health Care Team.
