Job summary

HCRG Care Group is recruiting a Band 6 Physiotherapist to join the Early Supported Discharge team at Gravesham Community Hospital, Gravesend. You will deliver high-quality rehabilitation in patients’ homes, supporting safe discharge and helping regain independence through a patient-centred approach.

You’ll work within a multidisciplinary team, including OTs, SLPs, and nurses, with opportunities for development and progression.

Qualifications

• Degree in Physiotherapy with HCPC registration.
• Post-registration experience in neurology, rehabilitation or community settings.
• Strong communication and ability to work in a multidisciplinary team.
• Experience managing a clinical caseload and delivering evidence-based interventions.

Responsibilities

• Assess and treat patients in the ESD service with neurological conditions.
• Manage a caseload and support safe discharge home.
• Supervise junior staff and students as Band 6.
• Collaborate with multidisciplinary colleagues and external partners.
• Use outcome measures to inform practice and maintain documentation.
• Contribute to service development and quality improvement.
