Job summary

Peter Bedford Housing Association (PBHA) seeks a Community Connector to work with residents facing mental health challenges, linking them to local services, housing, finance and employment opportunities.

You will be embedded in a multi-disciplinary neighbourhood team, coordinating personalised support plans and recording outcomes to drive positive change in Hackney and surrounding areas.

Qualifications

  • Ability to work collaboratively as part of a team across departments.
  • Willingness to support recruitment, selection, induction and training of staff and volunteers.
  • Commitment to training, supervision, performance reviews and team meetings.
  • Knowledge of PBHA policies including equality, diversity & inclusion, data protection, health & safety and finance controls.
  • Ability to plan, prioritise and organise workloads with minimal supervision.
  • Flexible approach to undertaking other reasonable duties.

Responsibilities

  • Provide one-to-one assessments in person, over the telephone or online.
  • Help people identify what is important to them, their interests, life goals and areas of support (housing, finance, health).
  • Assist people to identify wider issues impacting health and wellbeing (debt, housing, employment, loneliness, caring responsibilities).
  • Agree and monitor personalised support plans with people.
  • Ongoing assessment and management of risks with positive risk-taking.
  • Identify barriers to accessing services and help overcome them.
  • Work with Neighbourhood team and statutory mental health services.
  • Discuss referrals and feedback on outcomes from support provided.
  • Build knowledge of statutory mental health services and connect people to them.
  • Monitor and record outcomes and report activity and outcomes.
  • Collaborate to gather data and information for reporting.
  • Promote tenants/participants to PBHA’s participation and co-production opportunities.
  • Adhere to lone-working procedures.
  • Participate in key meetings.
  • Contribute to policy development, planning and external partnerships.
