Qualifikationen

• Experience in mental health or transferable skills within a similar field.
• Ability to work autonomously from detailed support plans.
• Willingness to provide services across multiple client needs with care and professionalism.

Aufgaben

• Deliver person-centred support promoting independence.
• Complete dynamic risk assessments and support plans.
• Promote wellbeing and manage risk.
• Adhere to company and national policies and procedures.
• Work flexibly including hours outside normal schedule to provide consistent support.
• Support people in their homes or accommodations and assist with safe discharges.
• Collaborate with a multidisciplinary team and maintain training.
