You will help clients move forward with independence and dignity, working with multidisciplinary teams to implement tailored care plans, monitor progress, and maintain clear records. We offer training, supportive leadership, and opportunities to grow within a recognised organisation.

• Provide person-centred support to individuals with complex needs.
• Assist clients in developing independent living skills within accommodation settings or their own homes, promoting dignity and social inclusion.
• Collaborate with multidisciplinary teams to implement tailored care plans, monitor progress, and manage risks effectively.
• Maintain accurate records and ensure compliance with safeguarding and health and safety standards.
• Engage with clients compassionately, fostering trust and encouraging positive outcomes.
