Job summary

• Strong clinical assessment skills and monitoring of risk in community settings.
• Ability to work autonomously within a multidisciplinary team.

Responsibilities

• Assess and manage service users with complex mental health needs in the community.
• Provide evidence-based interventions and develop personalised care plans.
• Coordinate care with families, carers and other professionals; support discharge planning.
• Deliver timely pharmacological and non-pharmacological interventions as required.
