Nottinghamshire Healthcare NHS Foundation Trust is seeking a Band 6 Mental Health Practitioner for its Ashfield Local Mental Health Team. The role focuses on assessing adults with severe mental illness, planning personalised recovery, and coordinating with partner agencies in a weekday multidisciplinary setting.

Applicants should come from a mental health nursing background and be able to manage complex cases with multidisciplinary support.

Qualifications

• Must hold a professional mental health qualification to degree level or equivalent.
• Practice-based educator qualification or willingness to train is required.
• Additional specialist or post-registration qualification in the service area is desirable.

Responsibilities

• Assess, plan and deliver recovery paths for adults with severe mental illness.
• Manage caseloads under the Care Programme Approach and coordinate with partner agencies.
• Carry out initial assessments and one-to-one interventions with service users.
• Record comprehensive risk assessments and devise treatment plans.
• Collaborate with other agencies to manage complex cases in the community.
• Maintain professional competence and supervision in a multidisciplinary team.
• Communicate effectively with service users, carers and health professionals.
• Defuse emotive situations during assessments and pathway planning in acute settings.
• Demonstrate high-level communication skills.
