We are pleased to offer the new and exciting opportunity to join the new Mid and South Essex, Older Adult Admission Avoidance & Home Treatment Team. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs. Valuing you. Recognising your dedication. • Receive supervision and support to help you fulfil your potential.
• Join an inclusive EPUT community and connect with others through engagement events and equality or champion networks.
• If you need help, we provide mental health and wellbeing services, occupational health advice and counselling., As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex
  27 days holiday, plus bank holidays, rising to 33 days after 10 years' service.
