ESSEX PARTNERSHIP UNIVERSITY NHS FOUNDATION TRUST is seeking a clinician to assist the senior team in delivering care and rehabilitation to patients at home after acute events, aiming to maximise independence and person-centred outcomes.
Work within a multidisciplinary team, support therapists, and contribute to care planning. The role includes infection control, supervision, and taking part in activity of daily living support for patients, with benefits such as 27 days holiday and NHS pension.
To assist the senior team in providing care and physical rehabilitation to people who require short team period of recovery in their own homes following an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live asindependently as possible., To work as part of the multidisciplinary team and when necessary on own initiative.To provide direct care and rehabilitation to the patients served by the team.To act in a manner that protects and maintains the patient's sense of dignity and respect, while observing their individual religious and cultural practices. Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.Ensure active involvement in manual handling of patients as required - in line with the Trust's Manual Handling Policy.To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.Observe record and report changes in the patients' health and environment to senior staff.To be familiar with the steps to be taken in medical emergencies and act accordingly.Complete observations including urinalysis and blood sugar monitoring as required. Assist the senior team with prelim visits, first visits and review visits to patients.Work directly with the therapists within the team to support them providing effective assessment and treatment including in the in-patient beds if necessary.Assist in planning work together with the senior team.Assist in contributing to care planning and risk management plans.
