Oxford Health NHS Foundation Trust is seeking a newly qualified or early career Occupational Therapist to join a brand-new Intensive Case Management and Rehabilitation Pathway across Buckinghamshire. You will work with adults and older adults in their homes and communities, delivering recovery-focused OT and helping people regain independence.

You will manage your own caseload with regular supervision within a supportive multidisciplinary team, contributing to discharge planning, risk management

Qualifikationen

• Full valid driving licence and access to a vehicle for work.
• Newly qualified or early career Occupational Therapist considered.
• Band 5 Occupational Therapist with caseload management is welcome.

Aufgaben

• Deliver high-quality Occupational Therapy assessments and interventions for adults with complex mental health needs.
• Support people to develop skills, confidence and independence to live in their communities.
• Provide assertive outreach by meeting people where they feel most comfortable, including homes and community settings.
• Build therapeutic relationships with individuals who may find traditional services difficult to engage with.
• Collaborate with colleagues across health, social care and voluntary services for joined-up care.
• Contribute to recovery planning, risk management and discharge planning.
• Maintain accurate clinical records and participate in service development, quality improvement and audits.
