Job summary

NHS Scotland is seeking two Community Mental Health Nurses with Older Adult Mental Health experience to join the Caithness Community Mental Health team in Wick. The post involves care for adults over 65 or memory concerns, including assessment, planning, implementation and evaluation of care with families and carers.

Travel and a full driving licence are essential; access to a pool car may be available. You will work within the MDT and may supervise students and Support Workers.

• Full driving licence is essential; own vehicle may be used for work.

Responsibilities

• Assess and plan care for adults over 65 or memory concerns.
• Implement and evaluate care plans with clients, families and carers.
• Monitor medications and liaise with Psychiatrists, GPs and Social Care services.
• Be an active MDT member; supervise students and Support Workers occasionally.
• Travel to deliver care and work across Caithness Community Mental Health services.

We are looking for two enthusiastic Community Mental Health Nurses with Older Adult Mental Health experience to join our Caithness Community Mental Health team based at Town & County Hospital, Wick.

Your role will be with adults over the age of 65 or of any age if a memory concern.

It will involve assessment; care planning, implementation and evaluation of the care plan, in partnership with the client, family and carers. You will also be involved in assessment and management of ongoing risk.

The role will involve medication monitoring, liaising with Psychiatrists, Primary Care and with other Social Care services, referring on to other services as appropriate. You will be an active member of an MDT and you will be supervising students and Support Workers on occasion.

The applicant would be required to travel efficiently and effectively to undertake the role.

A full driving licence is essential and a pool car may be available, however, applicants should have access to their own vehicle for work purposes, if required.

There are 2 permanent posts available totalling 49.2 hours, hours negotiable
