Job Overview

The Community Mental Health Team is seeking a skilled, motivated, organized, and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team. The focus will be on individuals with severe and enduring mental health problems. The successful candidate will be responsible for care coordination and case management of a defined caseload of clients with complex and challenging needs, as outlined in Parts 2 and 3 of The Mental Health (Wales) Measure 2010.

Main Duties of the Job

  1. Coordinate care for a designated caseload of clients with severe and enduring mental health problems.
  2. Ensure adherence to Care and Treatment Plans and meet standards in line with the Mental Health (Wales) 2010.
  3. Act as a point of contact for GPs and other services, providing advice, support, and signposting to appropriate agencies.
  4. Develop outcome-focused care and treatment plans, including comprehensive risk assessments for actual and potential risks.

The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
