An exciting opportunity has arisen to join a busy and passionate CMHT within the centre of Swansea as a CPN. The role will consist of working as part of a multidisciplinary team in undertaking the role of the Care Coordinator in the delivery of a high quality community mental health service under the framework of the Mental Health Measure (2010) Wales, whilst maintaining the Recovery ethos at the forefront of nursing care.

Main duties of the job

To assess, implement and evaluate agreed care and treatment plans for each individual on the caseload which has previously been agreed with the service user/carers. To work autonomously but under the indirect supervision of a Band 7 registered nurse/ team manager and in conjunction with other health colleagues in delivery of a supportive service for those people under the care of EIP

Detailed job description and main responsibilities

There will also be an expectation of the successful candidates to work autonomously but also as part of a team, which will involve mental health assessments and providing a point of contact for existing service users in the absence of their care coordinator. The role will also occasionally involve facilitating key clinics within the CMHT undertaking a range of physical health checks or medication administration inline with local policy.

Person specification

Qualifications

  • Registered Mental Health Nurse
  • Evidence of post registration study e.g. post graduate diploma or equivalent registration experience.

Knowledge

  • Ability to demonstrate clinical knowledge base and implications of current legislation i.e. Mental Health Act 1983, Mental Health (Wales) Measure 2010, Mental Capacity Act, Deprivation of Liberty.
  • Able to demonstrate a good knowledge of the current care and treatment for adults with Mental Health needs.
  • An understanding of the Clinical Governance agenda.

Experience

  • Experience of multi disciplinary and multi-agency working.
  • Experience of risk assessment and risk management
  • Experience at Band 5 or above in related clinical environment
  • Experience of leadership within a team

Skills

  • Able to demonstrate excellent communication and interpersonal skills
  • Ability to travel daily between sites and patient homes in a timely manner to meet the requirements of the role/service
