My name is Robin Sunderlandand I manage a fantastic community mental health team based in South Kirklees.
The Enhanced Pathway Team is acommunity mental health team operating in South Kirklees and as a Band 6 CommunityPsychiatric Nurse (CPN) you will be crucial to the MDT way of working andpivotal in delivering high quality care to service users in the community.
We have adopted the FlexibleAssertive Community Treatment (FACT) model of care for service users who arepresenting with increased risk and require a more intensive level of support.This feature allows care coordinators to involve the wider MDT in assessing andformulating daily risk management plans and makes the focus on risk a teamapproach.
This is primarily a carecoordination role and you will be responsible for managing a caseload ofservice users with complex mental health needs
We are looking to recruit anenthusiastic and motivated qualified nurse who can work in a flexible,innovative and creative way. The role will primarily be based at Folly HallMills Huddersfield and surrounding areas with a working pattern consisting ofMonday to Friday 9am - 5pm, we are however keen to support a good work and homelife balance so flexible working is available for all team members and wesupport hybrid ways of working.
Due to the large geographicalare that we cover being a car driver is essential, milage is available to claimvia e-Expenses
A key element of the role will be care coordination and MDT working undertaking initial and risk assessment, formulation and reviewing care plans of service user which promote engagement, stability, recovery, and risk management.
You will manage a varied caseload and function as part of a multidisciplinary team.
Work with service users with both short-term interventions and severe and enduring mental health needs to promote health and well-being.
Document care delivery to a high standard in line with professional standards, local and national guidance.
Recognise situations that may be detrimental to the health and well-being of the service user and initiates action that may be required to manage the situation safely.
Management of enquiries and direct referrals from other services such as Single Point of Access.
Support student education when allocated to the team and to contribute to the preceptorship and education of newly qualified staff where appropriate.
Maintain professional registration and demonstrate commitment to statutory code of practice and continued professional development requirements.

Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of a Core CMHT.
Management of own caseload in line with C.P.A. procedures and risk management protocols.
Provision of leadership to the Core Team in respect of psycho-social interventions to people and carers.
Supervision and support of other team members.
Participation in service development and clinical governance structures.
Assesses needs and identifies problems relevant to the care of people referred.
Devises a plan of care in partnership with the person and carers.
Implements the planned programme of care to ensure a high standard is achieved.
Reviews the effectiveness of the care provided and where appropriate initiates any action.
Advises and supports the person and carers to promote health and well-being and to prevent illness.
Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community.
Functions as a member of a multi-disciplinary team.
Management of enquiries and direct referrals of people in accordance with the Core C.M.H.T. Operational Policy and participates in the team duty rota.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
Responsible for acting up in the absence of the team leader.
Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
Participates in the formulation of policies at unit level.
