Job description

We are recruiting for a Band 6 Community Mental Health Practitioner (RMN) to join the West Lancashire CMHT, based at Bickerstaffe House, Ormskirk Hospital.

This is a full-time role working with adults with a variety of complex mental health issues, delivering care both from base and within the community. The successful candidate will be responsible for a caseload and will be expected to work collaboratively within a multidisciplinary team.

Key Responsibilities:
Conduct comprehensive mental health assessments and develop personalised care plans

Deliver evidence-based interventions and manage a varied caseload

Complete risk assessments and risk management plans

Liaise with other professionals to ensure coordinated care

Provide support in both clinic and community settings
