Job summary

Central and North West London NHS Foundation Trust in Brent is seeking a dedicated mental health professional to support adults in crisis, focusing on recovery and person-centered care.

You will undertake assessments, contribute to MDTs, provide supervision, and strengthen care pathways within CNWL policies and safeguarding frameworks. The role emphasises collaboration with General Practice Teams and community services.

Responsibilities

• Undertake initial and comprehensive assessment of health and social care needs, alone or with colleagues, and feedback to Multi-Disciplinary Team.
• Provide supervision to an identified group of staff and students on placement with the team.
• Carry out MDT activities including being on duty and clinic rota as appropriate and within sphere of profession.
• Develop and maintain effective communication strategies between Mental Health Services and General Practice Teams to improve care pathways.
• Work within CNWL policies and procedures, professional and legal framework upholding the values and ethos of the organisation.
• Maintain understanding and practice within Code of Professional Conduct, Adult and Child protection protocols, Mental Health and Capacity Acts.
• Provide regular ongoing contact with GP Practices as part of the link worker role, understanding the CMHT's role and function.
