Job summary

CNWL NHS Foundation Trust is recruiting for a Community Mental Health Practitioner to join the Brent CMHT. The role focuses on assessing health and social care needs, coordinating care, and supporting individuals experiencing mental health crises within a recovery-focused framework.

You will work with a multidisciplinary team, participate in duty rosters, and contribute to care planning and patient pathways while maintaining strong links with GP practices and community services.

Qualifications

• Accurate assessment of health and social care needs.
• Experience in mental health care pathways and MDT working.
• Ability to work within safeguarding and consent frameworks.

Responsibilities

• Undertake initial and comprehensive assessments of health and social care needs, alone or with colleagues.
• Provide care co-ordination and support for service users within a recovery and social inclusion framework.
• Supervise junior colleagues and students on placement with the team.
• Engage in MDT activities including duty and clinic rosta as appropriate.
• Develop and maintain effective communication with General Practice Teams to improve care pathways.
