East of England Community Health and Care NHS Trust is seeking staff to assist in delivering high‑quality, safe, person‑centred care in patients’ homes as part of the North UCR MDT. Care is provided over a 12‑hour period and the role operates within the 7‑day service framework.

We are an expanding urgent community response team delivering frontline community care and aiming to prevent hospital admissions and support early discharge.

Responsabilidades

• Assist in the delivery of safe, person-centred health care to patients in their own homes by following prescribed care pathways over a 12 hour period.
• Fully supported by the Urgent Community Response (UCR) Team with planning and delivering care for this complex group of patients.
• 7 days a week service; hours of service delivery 8am-8pm, 365 days a year.
• Contribute to the delivery of care pathways for patients with long-term conditions, palliative care and rehabilitation needs.
• Prevent unnecessary admission to hospital and support early discharge from hospital as part of an MDT.
• North UCR is a multi-disciplinary team of Nurses, Occupational Therapists, Community Assistant Practitioners and Healthcare Assistants; an adaptable team delivering frontline community care to patients in their own home.
