Band 7 Community Matron - 37.5 hours per week
North East London Foundation Trust has an exciting opportunity for a Community Matron in Romford Clinic, ICT Central.

The post holder will work as a complex case manager independently. Responsibilities include delivering integrated care in collaboration with GPs, Social Services, and third sector organizations, facilitating early discharge, and preventing unnecessary hospital admissions while supporting healthcare closer to home.
The service operates from 09:00 - 17:00, Monday to Friday. The post holder must support their team and organization in achieving the Trust’s values. Requirements include being a Registered Nurse with at least 2 years of community experience managing complex caseloads, utilizing advanced nursing skills such as assessment and prescribing.
They will work within and across professional boundaries, liaising with GPs, other healthcare professionals, and voluntary agencies to deliver personalized care.
Probationary Period
This post is subject to a probationary period. Internal applicants are exempt unless they are already in a probationary period or are bank staff.
Induction and Support
New starters will join on the first Monday of each month, undertaking a comprehensive two-week induction, including mandatory and systems training, equipment allocation, and engagement activities. Inductions are held at our Rainham head office.
High Cost Area Supplement – London
This role attracts a 15% supplement of the basic salary (minimum £4,551, maximum £5,735 per annum pro rata).
