Job summary

Daisy Chain Project is seeking a dedicated professional to support children, young people and adults up to 25 with learning disabilities or autism. You will manage a dynamic caseload, develop personalised plans with families, and coordinate multi-agency support to prevent crises.

You will work within a collaborative team, using creative communication methods and ensuring safeguarding at all times. The role is fixed-term for 1 year, 37.5 hours per week, with potential extension subject to funding.

Qualifications

  • Experience of working with neurodiverse individuals with complex needs.
  • Experience of working in the community to provide bespoke support to children, young people, adults and families.
  • Experience of working with children and young people at risk of inpatient admission.
  • Experience of multi-agency and joined-up working.
  • Experience of engaging with a wide range of professionals including at multi-disciplinary panels.
  • Experience of managing a caseload of families and providing behaviour management support in the community.
  • Experience of crisis management.
  • Substantial safeguarding experience.

Responsibilities

  • Maintain a dynamic caseload of children and adults up to 25 years with learning disabilities or autism identified on the Dynamic Support Register.
  • Develop personalised plans with families and deliver outcomes flexibly.
  • Provide bespoke support to build self-management skills.
  • Support parents to access and implement tools to manage behaviour.
  • Facilitate interventions to prevent admission to inpatient settings.
  • Use creative and accessible communication methods (pictures, role play) to engage the person.
  • Advocate for individuals and families to ensure necessary support is in place.
  • Coordinate with health, social care and education across teams to ensure continuity of care.
  • Maintain accurate records and monitor progress to adjust support needs.
  • Attend safeguarding duties and ensure policies are followed.
  • Promote equality of opportunity and maintain Daisy Chain branding in communications.
