Community Inclusion Lead - The Life Rooms - Band 6

The Life Rooms is one of Mersey Care NHS Foundation Trust's innovative services. Our Social Model of Health is designed to support the prevention and population health agendas through health activation and community access.

We have an exciting opportunity for an enthusiastic and driven individual to join The Life Rooms Team as a Community Inclusion Lead across Sefton Pathways Advice Service.

The Pathways Advice Service works with individuals to address the wider social determinants of health and their impact on mental and physical health and wellbeing. The service provides socially prescribed support tailored to individual needs, aiming to reduce health inequalities.

Main duties of the job

The Sefton Community Inclusion Lead will be responsible for:

  1. Managing the Sefton Pathways Advice Team.
  2. Managing the Wellbeing Support Team.
  3. Overseeing the Sefton Pathways Advice offer to ensure safety, quality, and effectiveness standards.
  4. Ensuring delivery of service specifications and meeting key performance indicators (KPIs).
  5. Engaging with Mersey Cares services to promote The Life Rooms offer.
  6. Building connections with partner organisations in Sefton to support social prescribing.
  7. Project oversight and development.

This is a 12-month fixed-term contract to cover maternity leave. The post is based at Southport Life Rooms with travel throughout Sefton expected.
