The Community Nursing Service in Cornwall offers an opportunity for a motivated Community Health Care Assistant to join the Toressa Community nursing team covering the East PCN. The role involves delivering nursing care to patients in their own homes, including those approaching end-of-life care. Planned nursing care may include wound care, venepuncture, catheter care, pressure area care and delivery of personal care to end-of-life patients.
We work in integrated ways to achieve the best patient outcomes. Previous care experience is essential. The Trust is committed to personal learning and development in community nursing. Travel to and from the base and patient homes within the geographical area is required and may not be served by public transport.
This position is currently open only to people who already work for the NHS in Cornwall or the Isles of Scilly.
To carry out assigned tasks with regular duties involving direct care to patients and carers to support the registered nurse; to work as a member of the community nursing team within a defined geographical area. The service covers from 8am to 10pm with 7.5-hour shifts.

Essential criteria: NVQ Level 3 in care or equivalent, GCSE Maths & English or equivalent, evidence of experience in care setting, trained in Venepuncture or willingness to undertake training. Desirable: additional relevant healthcare training.
Essential criteria: good communication skills, excellent verbal and written skills, ability to work without direct supervision and to time restraints, ability to cope calmly with stress and distressing situations, ability to cope under pressure.
