Job summary

Waythrough is seeking a Health and Wellbeing Nurse to support the clinical team in delivering direct service user care across Forward Leeds sites in Leeds. The role is full-time, 37 hours per week, with occasional late clinics and a permanent contract.

You will conduct alcohol assessments, support detoxifications, coordinate pathways with hospitals, provide BBV screening and immunisations, and promote harm reduction.

Qualifications

• Registered Nurse with NMC registration
• Experience in BBV assessment and care
• Experience in health promotion and harm reduction
• Ability to work across sites and coordinate with hospitals
• Interest in bridging health and equality
• Commitment to supporting vulnerable people

Responsibilities

• Provide alcohol assessments and community detoxifications, with referrals to inpatient or other networks
• Develop onsite service community alcohol detoxifications with operational colleagues
• Coordinate pathways with hospitals for alcohol misuse patients
• Provide healthcare assessment across services, including needle exchanges and outreach
• Coordinate BBV services including screening, vaccination, re-screening and treatment
• Coordinate and deliver call and recall services for Hepatitis B & Hepatitis A immunisations and HIV testing
• Promote harm reduction across needle exchanges and outreach services
