Job summary

Walsall Healthcare NHS Trust invites applications for a Band 3 Community Nurse to support the Community Nursing Service. You will deliver care to housebound clients in their homes or care homes, assist with clinical procedures, and follow care plans under supervision from a senior clinician.

Qualifications

• NVQ Level 2 in Health and Social Care or equivalent qualification and / or relevant experience.
• Ability to work within the lower level competencies as developed within the community locality teams.
• Current experience of working within health/social care setting.
• Ability to organise own workload and use own initiative.
• Ability to work effectively as part of a team.
• Ability to utilise relevant IT software e.g. Microsoft Word, e-mail.
• Effective communication and interpersonal skills with a proven ability to work with other professions.
• Awareness of local clinical policies and procedures.

Responsibilities

• Highlight risk and changes in patient’s circumstance/condition by effective communication with registered practitioners.
• Implement and deliver a care plan, undertaking assigned procedures as directed, work within boundaries identified by the registered practitioner.
• Demonstrate excellent communication with clients, relatives, carers and colleagues.
• Maintain written and electronic patient records.
• Undertake nursing procedures and treatments as required, within the role’s competencies and Trust protocols.
• Support and enable clients to maintain their health needs.
• Deliver health education and health promotion activities (e.g., smoking cessation, diet, exercise).
• Inform the senior clinician of all changes in relation to patient care.
