Job summary

The NHS Jobs Practice Nurse role delivers high-quality nursing care in the GP surgery and to housebound patients, working with GPs and the primary care team.

You will assess, plan, implement and evaluate patient care, provide health advice and vaccinations, manage chronic diseases, and support quality improvement across the practice. You will also work within infection control and information governance standards, and supervise learners where appropriate.

Qualifications

• Registration with NMC and adherence to the NMC code of professional practice.
• Registration qualifications appropriate for tasks and responsibilities.
• Commitment to maintain a high professional standard of nursing care and up-to-date practice.
• Awareness of personal limitations and experience.

Responsibilities

• Access the nursing needs of patients in the surgery, the community and housebound.
• Provide appropriate care and treatment in conjunction with the General Practitioners, or independently, as appropriate with and according to practice policies and protocols.
• Maintain effective liaison with other agencies and staff concerned with patient care and with other disciplines within the practice with appropriate regards to confidentiality.
• Assist in the formulation of practice philosophy, strategy and policy.
• General Nursing Care of the patients in the surgery and their own home.
• Wound Care Assessment and Management; Removal of Sutures; Venepuncture; Hypertension Management; Immunisations; Screening & Health Promotion.
• Provide care for chronic disease management including Diabetes, CHD, COPD, etc., and support vaccination programs.
• Safeguarding Chronic Disease Management; Home/Community nursing for chronic disease patients.
