Job summary

EPUT is recruiting for a Band 5 Community Nurse at Stansted Clinic in West Essex. The post involves assessing care needs, developing and evaluating care plans and delivering nursing care across a defined geographical area, including weekends and evenings.

You will work within Integrated Neighbourhood Teams supporting North Uttlesford PCN with a focus on prevention and discharge facilitation. The role requires NMC registration or progression, strong clinical skills, and community nursing

Qualifications

• NMC registered or working towards.
• Clinical skills and experience in patient care.
• Community Nursing experience.

Responsibilities

• Take responsibility for patients in their own homes or defined areas.
• Undertake comprehensive nursing assessments.
• Plan, evaluate and prioritise interventions and care.
• Lead clinical nursing management of the caseload's most dependent patients.
• Regularly review caseload patient care and inform Senior Nurse of changes.
• Prioritise workload and manage time effectively.
• Act up in Senior Nurse's absence.
• Deliver nursing care for a geographical area including weekends and evenings.
